Enrollment for the 2025-2026 School Year

Please note: ALL 4 forms must be completed/submitted and the $60 registration fees must be paid in order to secure your child's registration.  You can download the forms below and return them via email, mail, or they can be put in the mail dropbox next to our front door at the school.  

OR.....you can fill out most forms online by clicking on the dropdown menu above under More, Classes, Tuition, & Programs, then on each form.  Click on the  arrow  in  front of  Registration.  Make sure to click the SUBMIT button at the bottom of each form.

We are unable to make the Oregon State Immunization form fillable online. Please open and download below.

Registration Fees can be paid by cash or check, or you can request an invoice to pay online.
